Sacramental Certificates are required to receive additional sacraments, enroll in a Catholic School, get married among other things. If you need to show proof of receiving your sacraments, there are a few things you should know:
All your sacraments are recorded at your church of Baptism regardless of where you may have received your First Communion, Confirmation, or Marriage.
If you were Baptized at St. Matthew, we will be able to provide a certificate verifying that you indeed have received not only Baptism but any and all other sacraments you have received regardless of where.
If you have received your First Communion, Confirmation and/or were Married at St. Matthew, but not Baptized here, we would only be able to provide that specific sacrament(s) you received here as we are not your church of Baptism.

You will also notice that there are forms linked to this page. One is for an adult's information, and the second is for a child of whom the requestor must be a parent or legally responsible adult.
Choose the appropriate form, print it out and follow the directions. This form MUST BE SIGNED in order for us to process your request.
Once signed please attach a photocopy of your Driver's License or other legal ID as proof of identity. Without the identification, your request will be delayed until we receive it.
